PRE-EXAM PROCESSING

  STEP 1
Save the personal data form and privacy statement/medical release to your desktop. You may complete them on your computer or print them and fill them out in pen. You will need to include your FAA confirmation number from step 3. Once completed you will send them to us as noted in step 4.

STEP 2
Access to PEPP has been permanently discontinued from this site; therefore, you must complete the Report of Medical History (2807) via a hard copy DD Form 2807-1 in place of the on-line version. Since you will not have a user key to enter on the personal data form, please enter "CIV-DD Form 2807-1 attached"; we will then create the exam online for you.

Providing us a detailed medical history is very important. Please enter your personal demographics and answer all questions YES or NO. All yes responses must include specific information about the condition, dates, and diagnosis given.

When completing DD Form 2807-1 (medical history), you must answer each question truthfully. If you have answered YES to any medical conditions on a previous military physical, you must answer YES to those same questions, as well as entering any new medical findings. 

Intentionally falsifying medical documents could result in immediate disqualification of your IFCI physical.

Additionally, any history (birth to present) of the following medical conditions require you to complete the attached questionnaire. Once completed, please submit along with all other required documents as noted in step 4.
  ADD or ADHD       History of asthma 
  Allergies      Kidney stones
  Bedwetting      Motion sickness 
  Corneal Refractive Surgery (PRK, LASIK or LASEK)     Sleepwalking  
  Head injury or loss of consciousness      Tattoo and scar locations
  Headaches      Women: GYN (women only)


STEP 3
NOTE:  This step does not apply to IFCII (Flight Surgeon) applicants.

If you have had an FAA exam (any class) within the past 36 months, you do not need to complete this step; please continue to step 4. FAA Class III medical/student pilot certificates are now valid for 5 years.

 If you have never had an FAA exam, click here and register for a Third Class Student Pilot Medical Certificate, to be done in conjunction with your USAF FCI exam at USAFSAM.

The confirmation number provided through MedXpress is only valid for 60 days. The FAA deletes registration records after 60 days. For this reason, please update your registration record within 60 days of your FCI appointment date. We need to know that you're registered for your FAA exam when you submit your pre-screening package but we also need your FAA physical registration to be current/available after you arrive at USAFSAM during the week of your physical. Please forward new FAA confirmation numbers in the subject line of an e-mail to the MFS organizational e-mail box.

STEP 4

Please use the following format when sending your information
E-Mail subject line - Last Name, last four SSN, type of exam, and exam date scheduled.
EXAMPLE: Smith-4222-FC I- 4/12/2010

It is very important that you send the required information at least 30 days prior to your appointment - 45 days for those with PRK/LASIK surgery history. This will offer the best opportunity to avoid last minute delays in processing and possible requests for additional medical information which could delay or cause the cancellation of your exam all together. Please send all of your information together in one e-mail, if possible to avoid it being misrouted. We will review your documents in a timely fashion and create your physical exam case file awaiting your arrival. If we require additional information regarding your medical history, we will contact your program manager/Detachment NCO with further instructions.
